This policy explains how Form & Verity handles information in connection with this website and our related online services.

Course registration, accounts, and course delivery happen on a separate learning platform at learn.formandverity.com, and any notices presented there also apply to that platform.

1. Introduction

Form & Verity is a U.S.-based education and training brand. This marketing website presents information about our courses and training and lets you contact us. It does not process enrollments or payments itself; those functions occur on the separate learning platform and through its payment processor.

Form & Verity is an educational brand operated by Andrew Simmons Counseling, PLLC.

For purposes of this Privacy Policy, references to “Form & Verity,” “we,” “us,” or “our” may refer to Andrew Simmons Counseling, PLLC operating through the Form & Verity educational brand, where the context requires identification of the legal operator.

2. Information You Provide

Depending on how you interact with us, this may include:

  • your name and email address, and the details you write in a message when you contact us
  • account and course information you provide when you register on the learning platform
  • purchase and transaction details associated with an order
  • support requests and other communications with us

Please do not send confidential clinical information or sensitive personal information about a child through general contact channels.

3. Information Collected Automatically

Like most websites, our site and its hosting infrastructure generate ordinary technical information when pages are requested, which may include:

  • the pages or files requested, and the date and time of the request
  • basic server and security logs

This website does not currently run analytics, advertising, or tracking scripts. If that changes, this policy will be updated before those technologies are deployed.

4. Payment Information

Payments for paid offerings are processed by a third-party payment provider on the learning platform. Your payment details are submitted to and handled by that provider under its own privacy practices. Form & Verity does not receive or store complete payment card numbers.

We use information to:

  • operate and maintain the website
  • provide the courses and services purchased
  • respond to inquiries and provide customer support
  • administer transactions, including refunds
  • protect against fraud, abuse, and security issues
  • understand and improve our offerings
  • comply with legal, tax, and recordkeeping obligations

If we later offer email updates or newsletters, participation will be optional and messages will include a way to unsubscribe.

We do not sell personal information. We may share information in these categories, where relevant:

  • service providers acting for us, such as hosting, learning platform, email delivery, and support tools
  • payment processors, for purchase and refund handling

Sharing with service providers so they can perform services for us is different from selling personal information, which we do not do.

This marketing website uses only technologies necessary for the site to function and to be delivered securely. It does not currently set advertising or analytics cookies, and it does not build advertising profiles about visitors. The separate learning platform uses cookies or similar storage that are necessary for sign-in and session management.

If nonessential analytics or advertising technologies are added in the future, this policy and any consent mechanism required by applicable law will be updated before those technologies go live.

8. Data Retention

We retain information for as long as reasonably necessary for the purposes described in this policy, including providing services, maintaining appropriate business records, resolving disputes, enforcing agreements, and satisfying legal, tax, and accounting obligations.

We use reasonable administrative, technical, and organizational safeguards appropriate to our size and the information involved, and we rely on established service providers for hosting and payment processing. No method of transmission or storage can be guaranteed completely secure, and we cannot promise absolute security.

10. Children's Privacy

Our website and purchasing systems are directed to adults. We do not knowingly solicit personal information directly from children through this website. Our parent education content concerns children and adolescents, but that does not mean the service is designed for children to create accounts or submit personal information.

If you believe a child has submitted personal information to us, contact hello@formandverity.com and we will take appropriate steps to address it.

11. U.S. State Privacy Rights

Residents of certain U.S. states may have privacy rights under applicable state law. Subject to applicable law and where those rights apply, these may include the right to request:

To submit a request, email hello@formandverity.com with enough detail for us to understand and verify the request. We may need to verify your identity before responding, and we may decline a request where an exception under applicable law applies. Not every right described above applies to every business or every situation.

Form & Verity operates from Texas. Where applicable under Texas law, Texas residents may exercise the kinds of rights described in the section above using the same contact method. This policy does not assert that any particular statute currently applies to Form & Verity, as coverage depends on factors such as business size and activities.

13. Do Not Sell or Share

Form & Verity does not sell personal information, and does not share personal information for cross-context behavioral advertising or targeted advertising. Because these practices are not part of how we operate, we do not maintain a separate opt-out mechanism for them. If our technology or practices change, this section and any required opt-out will be revisited before that change takes effect.
